DV lottery disqualified for the photo? What actually fails entries

The State Department has repeatedly said that photo problems are the most common reason DV lottery entries are disqualified. The usual failures: not exactly 600 x 600 pixels, a photo older than 6 months or reused from a previous entry, glasses, shadows or a dark background, digital editing, and forgetting that the spouse and every unmarried child under 21 each need their own compliant photo.

Disqualification is usually silent

Entries are not rejected with an error message. A noncompliant photo simply voids the entry, or surfaces at the consular interview after selection, when the stakes are highest. Checking every family member's photo against the spec before the entry window is the only defense.

Will the DV entry form tell me if my photo is bad?

It validates basic file properties, not compliance. An entry can be accepted by the form and still be disqualified later for the photo.
