Images embedded in certain files, especially phone-scanned JPEGs and some IRS-generated documents, trip CEAC's image filter. The official fix, step by step

First, convert the file to PDF. Second, open that PDF and save it again, or use your system's print-to-PDF function on it, which flattens the embedded image into a clean new file. Third, re-upload to CEAC. This double-flatten resolves most cases.

If the error persists, do not keep retrying into a deadline. Submit the document through the NVC public inquiry form at nvc.state.gov/inquiry and note the upload error in your message. A related access error, 'The requested URL was rejected. please consult with your administrator', is part of the same acknowledged issue.

Your visa photo still has to be right

If the file that fails is your visa photo, make sure the photo itself meets the US immigrant visa spec: a square JPEG, 600 x 600 pixels minimum, plain white or off-white background, taken in the last six months, unedited.
