Can you reuse an old passport photo?
No. You cannot reuse the photo from your existing passport for a renewal: the new photo must have been taken within the last 6 months and must not be identical to the one in your previous passport. You can reuse a recent photo across different applications, for example a passport and a visa, as long as it is under 6 months old and meets each document's spec.
Why renewals reject the old photo
A renewal exists to update the document, including the likeness. Submitting the same image as your old passport is an explicit rejection reason, even if you look the same. Leftover prints from the same photo session as your old passport photo fail this test too.
What reuse is fine
Prints from a fresh session can be used across applications filed around the same time. If you had extra prints made recently for a visa, the same image can serve a passport application within the 6-month window, provided the specs match.

Only if that session was within the last 6 months and this is not a renewal of the same document. For a renewal, take a new photo.
Does the new photo need to look different from the old one?
It needs to be a genuinely new photo, not a copy of the old one. You looking similar is fine; the image being identical is not.
